How do I obtain a prescription to receive physical therapy? It is necessary that a patient seek a referral from their physician, physician's assistant, or nurse practitioner. This referral is in the form of a prescription stating the patient's diagnosis and the words "physical therapy evaluation and treatment." At this point the physical therapist may initiate care.

Will you communicate with my physician regarding my PT treatment? Synergy Physical Therapy therapists are in frequent communication with referring physicians. After your first physical therapy appointment a typed evaluation will be faxed to your physician. We will re-evaluate your condition approximately once per month of treatment and send progress notes to your doctor. Please let your therapist know if you have a follow-up visit with your doctor and we will ensure that a progress note awaits your doctor before your next appointment so that they are up to date on your physical therapy progress and current status.

How long will my visit last? A visit usually lasts 45 minutes to one hour and will be one-on-one with a physical therapist that is specially trained in outpatient orthopedic physical therapy. The typical treatment protocol for musculoskeletal disorders is 3 visits per week for 4 weeks. However, based on the patient’s diagnosis, the number of required visits in order to return the patient to their prior level of function may require more or fewer visits.

What kinds of exercises are done? The treatment plan may include a variety of exercise that focus on improving range of motion, strength, neuromuscular control, balance, coordination and endurance. The treatment plan is customized to each individual based on signs, symptoms, and findings from the initial evaluation.

Will my insurance cover these services and what forms of payment do you accept? Synergy Physical Therapy is on most insurance plans and we will file with your insurance. Our front office staff will confirm patient benefits as per their insurance contract and any out of pocket expenses will be determined prior to the visit. Insurance benefits include deductible and co-pays. For your convenience, we accept Visa, MasterCard, checks, and cash.

What if I have no insurance? No insurance, no problem. If you do not have insurance,or have a very high deductible, and would rather not go through insurance, we offer a $50 payment per visit and can develope a payment plan if necessary.
